When it comes to effectively managing a chronic illness, sometimes education can be the best medicine.
If you are living with depression and/or diabetes, you are not alone. In fact, according to the American Diabetes Association, nearly 21 million Americans have diabetes. According to Mental Health America (previously the National Mental Health Association) more than 19 million Americans are affected by clinical depression each year.
TriWest is here to help. Our Condition Management program is focused on educating TRICARE beneficiaries on how to improve their health management skills while living with a chronic illness.
The program is free to eligible TRICARE beneficiaries and their family members and does not affect other TRICARE benefits.
The Condition Management program also includes assistance with asthma and/or heart failure.
Once enrolled in the Condition Management program, a TriWest clinical health coach will:
- Schedule an appointment to call and review your health status
- Develop a customized educational plan based on your status and personal health goals
- Send a letter to your physician so he/she aware of your program participation
- Work with you to reach established goals
- Continue to help you as long as you want to participate and work toward achieving your wellness goals
