to print out the enrollment form (TRS Request Form, DD From 2896-1)
- Current TRS enrollees in TRICARE’s West Region who pay the monthly premium by electronic funds transfer or credit card can re-enroll
by faxing the TRS Request Form to TriWest at 1-866-441-8843 before October 1, 2007
