To register for automatic enrollment fee payments:

• Go to www.triwest.com
• Select "online payments"
• Download and complete the applicable form (registered users may complete the EFT or Recurring Credit Card forms online)
• Mail the completed form and your first three-month payment (check, money order, or credit card) to the address indicated on the form
