PCM Changes
TriWest has been working hard to build a large network of providers in the West Region. TriWest recognizes that some beneficiaries would like to change PCMs because theirs no longer meets the needs of their family, while others may need to change PCMs because theirs deciding to leave the network. It’s easy to change PCMs using the following guidelines.
PCM Leaving the Network
If your PCM is leaving the TRICARE network, you will receive a letter of notification from TriWest. The letter will explain that your PCM is leaving the network, and you have the option to choose a new PCM or have TriWest assign one to you. The large provider network that is in place makes it convenient to find a new PCM in your area.
Change Your PCM
You may change your PCM as long as the new PCM that you choose is accepting new patients and your request meets with MTF guidelines. To change your PCM
- Use the Provider Directory to find a PCM that is accepting new patients.
- Download and complete sections I, IV, and VI of the TRICARE Prime Enrollment application and PCM Change Form . TRICARE Prime Remote beneficiaries will also need to complete section V.
- Mail the form to the address below before the date specified on your letter.

You may also change your PCM by calling TriWest at 1-888-TRIWEST (1-888-874-9378). Hold times are typically longest early in the week and during the morning and lunch hours.
