Request a new Primary Care Manager (PCM) in two easy steps:
- Use TriWest’s
Prime Enrollment Opportunities tool to locate a new PCM near you.
- Request the PCM change online through the
Beneficiary Web Enrollment (BWE) site.

Change Form and submit it to TriWest at the address noted on the form.
